Welcome and Thank you for choosing to enroll in CD Image Statements.  FNBT BANK [hereinafter referred to as "the Bank"] has expanded its banking services to allow our customers [hereinafter referred to as "customer(s)"] to utilize the convenience and efficiency of Mobile Media technology, and the parties hereto desire to receive their banking account statement via compact disk. Customer understands that the terms, conditions, instructions and procedures set forth below are in addition to those that apply to any other accounts that customer currently has with the Bank or any other services customer obtains hereafter from the Bank.

NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, FNBT BANK and customer hereby agree as follows:

By your consent below, you agree that we may provide you with your banking account statements in Mobile Media format.

You must notify us of any change in your e-mail address. To change your e-mail address, you may call us at 850-796-2000 or send us an e-mail to fnbtmail@fnbt.com.

In summary, you will receive a monthly compact disk containing a copy of your statement and all items to include deposit slips, checks and deposited items.
